Primary Care Pharmacist– Taranaki Extended Care Team

Fixed term to 6 January 2024, Full-time available (will consider part-time applications)

We are seeking a Pharmacist to join our extended care team. You will work with patients, whānau and their healthcare providers to deliver a clinical pharmacist service that improves medicines’ therapeutic effectiveness. If you are a person passionate about supporting individuals and whānau on their hauora journey, enhancing their own capacity to take care of their wellbeing, then this could just be the role for you.

 

As a key member of the extended care team of pharmacists, dietitians and social workers, you will be contributing to the collective team knowledge and support for the wellbeing of our community. In return for your professional skills and innovative outlook, we’ll provide you with an environment that allows you to professionally and personally flourish and you’ll be joining our team of clinical pharmacists across the Pinnacle Midlands Health Network who will provide collegial support.

 

To be successful in this role you will have:

  • Be a NZ registered pharmacist holding an annual practising certificate with no restrictions, and current indemnity insurance.
  • A postgraduate diploma in clinical pharmacy or equivalent (or working towards)
  • relevant clinical/community pharmacy experience
  • a passion for improving patient health through evidence based best practice
  • demonstrated knowledge and experience of Māori models of health and wellbeing
  • a keen ability to develop and maintain key relationships within a multi-disciplinary team, external agencies and supports.

 

This is a fixed term, full-time position, however part-time hours would be considered.
